Forms to print, fill out and return
Maryland requires these three forms before your child's first day. Download each one, complete it, and bring it back to the center — or email it to us ahead of time. Your child's doctor will need to sign the health inventory and lead testing certificate.
Emergency Form (OCC 1214)
Emergency contacts, authorized pickups and your child's medical release. Parent/guardian completes and signs.
